The Sequoia Project is a membership-based internal medicine practice serving adults age 19 and older. We provide proactive, evidence-based primary care and augmented management of lifestyle practices.


MEMBER BENEFITS INCLUDE  


—A comprehensive yearly exam focused on disease prevention and screening

Periodic health check-ins 

—Standard 30-60 minute appointments, longer appointments as needed 

Same and next day appointments for acute illness

—Texting and phone calls with your physician, including for after-hours issues

Virtual visits, including when you travel out of state  

Electronic medical records with a patient portal and online scheduling 

Sequoia Project offers an individual monthly membership at $285 per month for adults age 19-64 and $335 for patients age 65+.


Sequoia Project is out of network for all commercial insurance plans. Per Medicare requirements, we bill Medicare plans for any covered medical services. Monthly membership fees are not reimburseable by commercial insurance or Medicare. However, we work with your insurance to ensure specialty referrals and diagnostic testing are covered to the extent allowed.  


You may cancel your membership at any time with thirty days written notice. After cancellation, we require a waiting period before you may rejoin


HIGHLY INTEGRATED CARE COORDINATION: 


Specialist referrals: When medically indicated, your physician will involve medical and surgical specialists in your health care. She communicates closely with specialists with whom you have existing relationships, and can also refer you to consulting physicians close to your home who are a strong fit for your needs. She works with specialists in both large systems and small practices throughout the Seattle and Eastside areas. In certain cases, she can help coordinate care outside of the metropolitan area. 

Hospitalization: If you require hospitalization, you would be admitted to the hospital of your choice. Your physician will communicate and coordinate care with the attending hospitalist overseeing your care. 

Members will want to maintain health insurance coverage for any diagnostic and imaging tests, specialty consultations, medication coverage, and emergency & hospital care. Be aware that certain types of health insurance may be more likely to cover imaging tests or specialist visits ordered by an out-of-network provider.

PARTNERSHIPS

Sequoia Project partners with physical therapists, exercise physiologists, athletic trainers and dieticians. Members who are so inclined may elect to obtain assessments of lean mass, aerobic capacity (VO2 max), and metabolism. This information augments your personal health plan.
