Providers requesting medical records, please fax your request to 206.212.1100 or send by U.S. mail to 3216 NE 45th Place #301, Seattle, Wash. 98105, along with any appropriate documents. We are unable to respond to email or telephone requests for medical records.      

Location and parking: We are located near University Village in the Lakeview Medical-Dental Building. Two-hour parking is available along the northeast and west aspects of the building.
